How are you operating during the COVID-19 pandemic?
Since summer 2021, I have been conducting appointments both virtually or in my office in Perkasie, your choice. If COVID rates increase during the fall, I may significantly reduce in-person appointments and meet mostly virtually. We can wait and see!
Are there any forms I should fill out before my first appointment?
Once you schedule your first appointment as a new client, you will receive an e-mail to join our client portal. Within our portal, you will be directed to fill out new client registration forms and health history forms electronically prior to your appointment. You will also complete a 3 day food record before your first appointment.
How often will we meet in your program?
We will usually meet every 2-3 weeks, but you have the option to meet with me as much as possible! I only care about you feeling supported throughout your journey! You will also be able to utilize your portal to communicate with my Functional Dietitian Coach as much as you would like to, in addition to recording your food intake for my review and feedback.
I don’t live close by. Can we meet through video or phone?
Sure we can! Our client portal meets HIPAA regulations and is a secure spot for sharing sensitive information and meeting virtually.
Can I just get a test completed without being a client?
No, I only offer testing within my 1:1 counseling option
Occasionally, I will offer testing outside of nutritional programs. Join my e-mail list so that you get notified.
What should I expect to pay? Can I use my insurance?
I work with new, individual clients over FIVE month commitment only. There is an out of pocket investment, regardless if I accept your insurance or not. This investment covers the testing (not covered by insurance), coaching, test interpretation, protocol development, and all of the other work I do behind the scenes to make sure you have everything in order to get the results you are looking for.
I accept Aetna and Blue Cross plans (IBC, Highmark, BCBS). I will bill your insurance for the appointment portion of the program if it’s accepted.
The out of pocket portion of the program is collected at registration (50%) and at your first appointment when we have test results and can get started (50%). I do offer monthly payment plans as well.
Out of pocket investment varies and ranges from $4200-$5497 for the 5 months depending on your coverage. I am working on group program options right now so that if you have a lower budget, I can still meet your needs, so stay tuned!
The procedure code for medical nutrition counseling is 97802 and 97803. If needed, my National Provider Identifier (NPI) is 1427306034.
