Are there any forms I should fill out before my first appointment?
Once you schedule your first appointment as a new client, you will receive an e-mail to join our client portal. Within our portal, you will be directed to fill out new client registration forms and health history forms electronically prior to your appointment. You will also complete a 3 day food record before your first appointment.
How often will we meet in your program?
We will schedule 8 appointments over the course of 4 months.
During the beginning of your program, we will meet every 1-2 weeks for the first 2 months. Our appointments are centered around a full nutritional assessment (the Share Your Story appointment), In-Depth Diet Planning, and 2 hour lab review session and protocol strategy appointment. Then, we stretch the appointments out every 2-3 weeks as you move through your plan.
You can choose to meet virtually over phone or video or opt to visit me in my Perkasie office. Most of my clients mix up their sessons – some virtual and some in person.
I don’t live close by. Can we meet through video or phone?
Sure we can! Our client portal meets HIPAA regulations and is a secure spot for sharing sensitive information and meeting virtually. I am currently licensed in the state of Pennsylvania. There are some states outside of Pennsylvania that I can work with – just ask!
Can I just get a test completed without being a client?
Testing is only offered in my 1:1 program.
One exception, is the Micronutrient Testing Bundle. Please inquire about this if you are interested.
What should I expect to pay? Can I use my insurance?
The total cost for my 4 month program is $3597.00. This price is all inclusive which means Gut Zoomer (stool) and DUTCH Plus (cortisol and sex hormones)are included. My preference is to use testing so we can jump straight into your root causes (get best results this way!).
That being said, I do offer this program without testing for $1997.00 and happy to add testing on if you decide at a later point during your program.
The out of pocket portion of the program is collected at registration (50%) and the other 50% collecte 1 month later. I do offer monthly payment plans as well.
As of April 2023, I am no longer accepting medical insurance and am considered an out of network provider. I can provide you a super bill for each appointment. You can submit a super bill to your insurance for reimbursement if your insurance covers nutrition counseling.
